The people with an institution behind them were issued a credential and a number. Everyone else exists in whatever photograph somebody happened to keep. One file has no subject at all.

He moves through wards after the attendings go home and reads charts nobody thinks to stop him reading, because nobody sees him reading. He notices the pattern before the institution does. He is not believed.
